Routine Home Care
Routine home care includes scheduled, routine visits in the home by a multidisciplinary care team including hospice physician, registered nurse, hospice aide, social worker, chaplain, volunteer and bereavement specialist. These services are provided at the patient’s residence.

Continuous Home Care may be necessary when a patient experiences a medical crisis or when their symptoms require a more intensive management providing 24-hour per day care to the patient’s bedside.

If a patient’s symptoms management is ineffective and can no longer be managed at home, inpatient care provides round-the-clock care in a Medicare-certified hospice bed in the hospital, a dedicated hospice inpatient facility or a nursing facility. When the symptoms stabilize, patients return to their homes and routine care.
